Seems someone is challenging draft of final report. I suspect human error features highly

The review of the final report doesn’t seem to be making much progress. The final draft of the final report was ready 12 months ago.

“The review board is chaired by Senior Counsel Patrick McCann, who is being assisted by an independent expert member.

It is understood that witnesses have yet to be called before the board.

A spokesperson for the department said the review was entirely independent and that it was a matter for the chairman to determine how the process would be carried out and how long it would take to complete.”

Has it ever been established conclusively who called for this review? That (and previous) RTE reports suggests it is one of the parties directly involved in the investigation, which suggests eg the operator, or the IAA, as opposed to the family of a crew member, who would be entitled to call for a review also.

No. I doubt it was any family members. IALPA, the pilots union, issued a strong criticism of the delay last March.

“This review does not comply with the standards and recommended practices laid down by the International Civil Aviation Organisation (ICAO). Annex 13 of ICAO sets out how States should investigate or delegate the investigation of accidents which have occurred in their territory.'

'For a country to take the unusual step of deviating from ICAO practice it must file a difference to the ICAO Standards and we do not believe Ireland has taken this step.'

He concluded: "It is fundamental that adherence to international obligations regarding aviation safety recommendations should transcend stakeholder concerns over reputation. Prevention of any future accidents demands no less.”

am more interested in the cost of this report ??? and who will be paying for it....as opposed to whom requested it..

All you seem interested to know re who will pay for this is very easily answered - you will, as well as every other unfortunate who just keep on payin'!

Should it not, for once, be the schmucks then whoever commissioned it is he who pays the piper.

Either which way, you'd be better advised to veer into higher order question territory.
